This form has been developed to assist those who have been licensed under the above legislation to comply with the following. Please ensure that you complete this form correctly and provide the required items on submission.
Duties in relation to a Private Hire Operator, Private Hire Vehicle and/or Private Hire Driver
- • Condition 3 – Private Hire Operator's Licence
- • Condition 8 – Private Hire Vehicle Licence
- • Condition 11 – Private Hire Driver's Licence
Requires any holder of the authorisations listed above to 'notify the Council in writing of any change of his address during the period of the licence within 7days of such a change taking place'.
In terms of the Operator's Licence this also relates to 'any address from which he operates or otherwise conducts his business as an operator'.
There is an administration fee of £12.50 to cover the cost of amending our records and re-issuing the paper-part for each licence.
Duties in relation to a Hackney Carriage Vehicle and Hackney Carriage Driver
Under Section 44 of the Town Police Clauses Act 1847 it is a legal requirement to notify the Authority who has granted the licence with notification of a change of address within 7 days of such a change taking place.
We would also ask, out of courtesy, that if you have been granted with a Hackney Carriage Driver's Licence by this Authority, that you notify us within 7 days of such a change taking place.
There is an administration fee of £12.50 to cover the cost of amending our records and re-issuing the paper-part for each licence. The fee will apply to each licence that needs changing.
Change of Name
If you have formally changed your name then you must advise this Authority within 7 days. Under these circumstances the Driver's Badge will need to be replaced and will cost an additional £10.50.
