Apply or renew a carer permit

Introduction
1. Before completing this eform please visit our Carer Permit webpage to check your property is included in a controlled parking zone.
  
  To use this form to apply or renew a permit, we will need a copy of the following:
  
  • Proof of residency dated within the last 3 months.
  
  • Proof of Care letter from the care company stating how frequently the resident is receiving care.
  
  • Proof of Care – Bank Statement (showing payment for care)/Support Plan/Invoice for care payments.
  
  If care is being provided by an informal unpaid provider such as a partner, family member or friend please supply a copy of the following:
  
  • A letter from their doctor, on headed paper, stating that the applicant must receive either 24 hour care or continuous/daily care at their address due to their condition (the type of condition or disability need not be disclosed).
  
  Without the correct documentation your application will be rejected.
